Case Management
Our Case Management staff works closely with patients, families, physicians, and other medical personnel to assist with pre-admission planning, social evaluation, discharge planning, and coordination of referrals to other health, community, and support organizations. The Case Management Department is a resource for in-home community support services, home health agencies, as well as long-term care and assisted living facilities in the area. Staff members can assist patients and families acquire needed adaptive equipment such as walkers, hospital beds, or lift recliners through various medical supply companies or lending groups. We strive to make each patient’s discharge planning complete, meeting the physical, medical, and emotional needs of each individual.
The Case Management staff provides education regarding Advanced Directives, including Durable Power of Attorney for Health Care, Living Will, Guardianships, and Do Not Resuscitate requests. The department provides a choice in Advanced Directive documents, assisting patients to either contact their own attorney or helping patients formulate the Durable Power of Attorney for Health Care and Living Will documents of their choice.
Brodstone’s Case Management Department serves as a Community Partner for the Department of Health and Human Services of the State of Nebraska. We are able to help patients explore and apply for State of Nebraska Assistance Programs including Medicaid, Supplemental Nutritional Assistance Programs as well as Energy Assistance. Our staff will help coordinate available benefits to assist families to meet their basic needs. We provide assistance with Social Service Disability applications, as well as information on Social Security Retirement benefits and Medicare information.
We have Case Management Workers trained as Certified Application Counselors for the Marketplace Health Insurance Plans. Our staff can guide or assist you with the application process for insurance plans through the Marketplace.
Our department provides information regarding Drug and Alcohol counseling and treatment options, as well as mental health providers in our area. Current information on after-treatment programs and support groups is also available.
